TERMS AND CONDITIONS

PATHLAB GOLDEN DRAGON CONTEST 2024

PATHLAB GOLDEN DRAGON CONTEST 2024 is a contest organized by Pathlab & Clinical Laboratory (M) Sdn Bhd ("Pathlab"), inviting Pathlab customers who undergo health screening or make a purchase with a minimum value of RM100 to win a Gold prize. Participants are requested to read these Terms and Conditions carefully. By participating in this Contest, Participants have agreed to all the Terms and Conditions that apply to the Contest.

A. CONTEST PERIOD
Contest Period: 1 May – 31 December 2024 until 23:59:59 MYT

B. PARTICIPANT REQUIREMENTS

• Contest participants are consumers who make transactions at Pathlab with a minimum value of RM100 at Pathlab Malaysia during the period. This includes health screening, health supplements and/or any related packages purchased at Pathlab.
• Participants must have valid official identification (NRIC/MyKad and/or Passport).
• Participants are required to have a cellphone/cellphone number and not change their cellphone/cellphone number during the contest period.
• Employees of Pathlab Malaysia and parties related to this contest and their families; including advertising agencies, suppliers, agencies and third parties are not eligible to participate in this contest

• Prize distribution will be carried out using the scheme and transaction period according to the following table:

E. MECHANISM:

• Contest organized by Pathlab Malaysia (Organizer)
• Contest participants will get automatic entries for contest by making transactions with a minimum value of RM100 at Pathlab branches in Malaysia.
• The number of entries obtained will increase along with the increase in transaction value according to the following table:

• Participants do not need to fill in data because the data will be automatically recorded in the Organizer's system when participants register to purchase health screening / health supplements / related packages at Pathlab.
• If declared a winner, the participant will be asked by the Organizer to show proof in the form of an ID card and/or passport as well as proving an active handphone number as validation for the prize claiming process.
• Participants can make as many transactions as possible to increase their chances of winning gold prizes because number of entries will continue to accumulate and participants can win many times during the contest period.

F. CONTEST ENTRIES SIMULATION

Simulation 1

• Customer A underwent health screening once on May 1, 2024 worth RM1000; then customer A gets 30 entries.
• In the following month (12 June 2024) customer A will undergo another health screening worth RM3500; then patient A gets 190 entries. Patient A's accumulated entries until June was 220 entries.
• Until the end of December, the customer did not make any transactions.
• Customer A's total number of qualified entries until the end of December is 220 entries that can be entered for the Grand prize

Simulation 2

• Customer B underwent a health screening once until the end of May worth RM1600; then customer B gets 41 entries.
• At the announcement in June, Customer B won gold worth 3 grams in the monthly draw.
• In the following month, customer B will undergo another health screening worth RM600; then customer B gets 11 entries. Customer B's accumulated entries until June was 52 entries. Customer B will not be entitled for anymore monthly draw but will still be entitled for the Grand prize draw.
• Until the end of December, customer B did not make any transactions.
• The total number of customer B qualified entries until the end of December is 52 entries that can be entered for the Grand prize

G. SELECTION OF WINNER

• Winners are selected randomly and based on the entries obtained.
• Winners must fulfill the terms and conditions determined by the Organizer as contained in the mechanism section. If it is proven that they do not fulfill the requirements, then the winner will be disqualified and the Organizer will conduct a re-draw to determine a new winner.
• Winners will be announced in the first week of the following month.
• Winners will be announced via the organizer's official Facebook account, namely @mypathlab and will be contacted by the respective Pathlab branch outlets.

H. VERIFICATION & ANNOUNCEMENT OF WINNER

• For data collection purposes, the handphone / handphone number registered must be confirmed to be correct and always active for winner verification purposes.
• The selection of winners will be carried out in accordance with section G. Selection of Winners
• Winners will be announced in the first week of the following month.
• The winner will be contacted by the organizer for verification within 5x24 hours (working days) after the winner is announced
• If the winner cannot be contacted within 5x24 hours (Working Days), then the winner hereby agrees to waive his or her right to receive the prize as a winner and will release the organizer from demands or claims for awarding the prize in the future.
• After receiving the announcement from the organizer's official Facebook account (@mypathlab), the winner will be asked to come to the branch designated by the organizer bringing their personal identity card and/or documents, other supporters within a maximum of 7x24 hours (Working Days). If within this time limit the winner cannot submit completeness of the data, the winner hereby agrees to waive his or her right to receive the prize as a winner and will release the organizer from demands or claims for awarding the prize in the future.
• If the winner is unable to receive the prize directly, the winner must provide a Consent Letter to receive the prize from the person appointed by the winner. When receiving the prize, apart from the Winner's original NRIC/MyKad or passport, the recipient's original NRIC/MyKad or passport must also be provided.
• Before signing the prize receipt letter/form, the recipient of the prize, namely the winner concerned or the proxy, is required to double check to ensure that the item received is a prize that matches the Winner's announcement information and ensure that the prize is in good condition/not damaged.
• If the package check and receipt have been carried out and signed, then the gift recipient has agreed that the package received is an appropriate gift and if you submit a complaint after this process, the complaint cannot be processed.
• Each participant guarantees that any information provided is true and correct.
• The organizer is not responsible if errors/negligence occur which are beyond the organizer's control, such as: participant errors in providing information, and errors/negligence of other parties.
• All costs, whether personal costs or other expenses related to the prize confirmation process such as: transportation, lodging, are the sole responsibility of the Participant. Participants are fully responsible and bear full responsibility if undesirable things happen that are beyond the organizer's control, such as:

i. Accident
ii. Damage
iii. Lost their lives during the journey.

I. OTHER PROVISIONS

• The Organizer requires and collects the Participant's personal data to process the Participant's participation. By participating in this program, participants agree/permit the organizer to collect, use and process all personal information for the purposes of this program and including the requirements for receiving prizes from the organizer
• The organizer has the right to use winner data for program purposes
• This program is implemented without charging any fees. The Organizer is not responsible for any form of criminal act committed on behalf of the Organizer using communication media outside the Organizer's official Facebook account (@mypathlab).
• The organizer has absolute authority to disqualify participants who are found or suspected of cheating, falsifying verification codes, identities and handphone numbers to get prizes from this program
• The organizer has the right to disqualify participants who do not fulfill one or all of the terms and conditions of this contest. All participants must fulfill the terms and conditions set by the Organizer
• The organizer's decision is final and all forms of communication will not be entertained after the contest period ends
• The organizer has the right to change and/or modify the terms and conditions of the program from time to time with or without prior notification
